Tall Man lettering is a patient safety strategy endorsed by the FDA and Institute for Safe Medication Practices (ISMP). It involves capitalizing portions of drug names to highlight differences between look-alike, sound-alike (LASA) medications (e.g., hydrOXYzine vs. hydrALAZINE).

As stated in the ISMP Guidelines:

“Tall Man letters are used to help distinguish look-alike drug names and reduce medication errors, particularly during selection from computer screens or bins.”
